The log looks incomplete, it seems you put the "error_log" directive with
the debug option not in the main level of configuration, but in some level
below.  As a result it didn't catch all the events.

Please also provide your configuration.

At the first glance it more looks like some client bandwidth limitation.
Could you describe your environment and how do you test it?
