avoiding disk cache on proxy ?

You may use

      proxy_buffering  off;

or

      proxy_max_temp_file_size 0;

But backend will be bound up with client during whole download time.
If nginx and backend are on the same host, it's better to use
the "X-Accel-Redirect" header.
