[PATCH 0 of 2] HTTP/3 Insert Count Increment delay

After an internal discussion, here's a new patchset.  The first patch is now
different.  It makes sure that all functions that operate on a stream
receive stream connection, and all functions that operate QUIC connection-wise
receive QUIC connection.

As a side effect, logging seems to be more consistent now.  Connection-wise
operations are logged in parent connection log.
