I think it would be best if you turn on debugging / error logging on  
your server and show a sample of what files aren't being hit by the  
rewrite -- and where they should be served from instead on the disk path

wordpress out of the box is less than fun to set up and configure.  
the docs are a mixing of every version ever released, and their bug  
support system routinely ignores a lot of issues.

I think you'd find it a lot easier if you just listed what the  
rewrite args should be based on your server logs.  there's a very  
good chance that is more accurate of a description than any sort of  
documentation you got with wordpress.
