500 error on multipart POST, no error log

Wild guess... try without the quotes?  e.g. change
   location ~ "^/" {
               proxy_pass   http://127.0.0.1:5001;
       }

to
   location ~ ^/ {
               proxy_pass   http://127.0.0.1:5001;
       }
