nginx mp4 streaming module

I adapted the lighttpd mod_h264_streaming to nginx module. It is under GPL
license.

I tested it under:

Leopard 10.5.5
CentOS 5.2
FreeBSD 7.0

both 32-bit and 64-bit environment.
