Is there a third interface on the centos box with 151.1.210.45 ip?
Is it being NATed to the 192.168.x address?

As with the 192.168.x the server seems to send the response while the "outside" doesn't or you just left it out?
Anyways this doesn't seem to be nginx related and you might want to look up on the product support/docs:
https://www.ossec.net/docs/faq/unexpected.html#agent-won-t-connect-to-the-manager-or-the-agent-always-shows-never-connected
