problem with proxy pass

Is there a third interface on the centos box with 151.1.210.45 ip?
Is it being NATed to the 192.168.x address?

>From the provided tcpdump it's a bit unclear which case does work and which doesn't? 
As with the 192.168.x the server seems to send the response while the "outside" doesn't or you just left it out?
