[PATCH 0 of 3] HTTP/3 Stream Cancellation and friends

The series implements some improvements in HTTP/3.  On top of them Stream
Cancellation send is added.

- patch #1 improves throwing stream/connection errors
- patch #2 adds connection reuse and delayed request allocation
- patch #3 adds Stream Cancellation send
